2025 Required Registration Forms

2025 FALL PAPERWORK

To be collected in July at the Port-au-Peck Firehouse in Oceanport. 

  • Participant Contract/Parental Consent Form - Fill out page 2 of this form.
  • Medical Info Form - Filled out and signed by Parent
  • Medical Clearance Form -Filled out and signed by Doctor.
  • One school picture or wallet size photo. Must also be uploaded when registering.
  • Copy of school report card (year-end report card/4 quarters).
  • Copy of birth certificate
  • Undated check #1 for $150 for equipment deposit (tackle and cheer)
  • Undated check #2 for $100 for work bond (tackle and non-flag cheer)
