One school picture or wallet size photo. Must also be uploaded when registering.
Copy of school report card (year-end report card/4 quarters) **Incoming kindergarteners do not need report card**.
Copy of birth certificate - All fields must be visible
Undated check #1 - $150 for equipment deposit (all cheer) -(will not be cashed unless uniform is not returned in November)
Undated check #2 - $100 for work bond - EXCEPTION FLAG - (will not be cashed unless volunteer hours are not fulfilled. # of required hours TBD)
Fundraising participation per child (all cheer)
